The system does not succeed at its intended task in operation, or succeeds unevenly across subpopulations and input types, falling short of what the use requires. Measured ex post against the task the system was deployed for. Whether the system was validated for that task in the first place is fitness-for-purpose (Governance); for systems whose output is an evaluative score, whether the score measures what it claims is measurement-validity.
